Certificate of Required School Attendance (DMV Form)

The Department of Motor Vehicles now requires a Certification of Required School Attendance form to be submitted when you apply for a learners permit or obtain a driver’s license. Your Driver Education instructor may also require this. You will need this if you lose your driver’s license and try to get a replacement.

Please make arrangements to come by and pick up your form from either the guidance department or the high school office if you have plans for this. This should be requested very near the date you plan to obtain the license or permit. The DMV will not accept an old form so there is no need to get it too far ahead of time.

You will not be able to get this form from anywhere else or have it emailed because it must be signed by a school representative from the high school office or guidance department in blue ink to verify your school enrollment status.

Please remember to keep in mind school holidays and summer break in your needs. When our offices are closed there will be no one here to give the form to you. Summer needs and holiday needs should be addressed before the student leaves campus on holiday schedule or Summer Break.

Students may email pwilliams@chspanthers.com or rgonsoulin@chspanthers.com to let them know ahead of time that the form is needed. This will give time to prepare it and have it ready for pickup. Mrs. Ronilaine Gonsoulin is located in the high school office and Mrs. Patrice Williams is located in the guidance office (200 Hall near chapel).
